How the Immediate Reconstruction Process Works for Moh's Surgery

After a patient is diagnosed with skin cancer, the patient is seen for consultation by Moh’s surgeon to plan for excision of skin cancer. If patient would like to have the closing of reconstruction of wound created by Moh’s surgery to be done by plastic surgeon, a consultation is requested to be seen by plastic surgeon. Once patient is seen by plastic surgeon, the Moh’s surgeon and the plastic surgeon setup a date where both surgeons are available to perform skin cancer excision and reconstruction on the same day.  The treatment typically begins with Mohs surgery (or another skin cancer removal procedure) performed by a Mohs or skin cancer surgeon. After successful Moh’s surgery, patient is sent to Plastic surgeon for reconstruction of the wound.

Both the Mohs surgeon’s office and the plastic surgeon’s office coordinate to select a date when both surgeons are available. On the day of surgery, the Mohs surgeon first removes the skin cancer. The patient is then sent—often with a protective dressing—to the plastic surgeon the very same day. The plastic surgeon performs reconstruction and closes the wound immediately on the same day.

Because the wound is repaired on the same day as Mohs surgery, this is called Immediate Reconstruction.

In contrast, when the wound is left open for several days or even a week before being closed by a plastic surgeon, the process is referred to as Delayed Reconstruction. We offer both methods of reconstruction.

Aesthetic and Medical Benefits of Post-Mohs Reconstruction:

Post-Mohs reconstruction offers both functional and cosmetic advantages. It helps restore natural facial or body contours while protecting and preserving important anatomical structures. By supporting proper healing, reconstruction also reduces the risk of complications. In addition, advanced techniques are used to minimize scarring and create a more refined, natural appearance. Beyond the physical benefits, patients often experience renewed confidence and improved emotional well-being.

What to Expect from Reconstruction:

Your reconstructive care begins with a thorough consultation, where Dr. David Sahar will evaluate the wound, review your medical history, and discuss your personal goals. Depending on the complexity of the case, reconstruction may be performed immediately after Mohs surgery or scheduled as a staged procedure.

Surgical options may include:

  • Local tissue rearrangement – Nearby skin and tissue are carefully repositioned to close the wound while preserving natural shape and contour.

  • Skin grafting – Healthy skin from another part of the body is used to cover and heal the affected area.

  • Regional or free flaps – For more complex cases, larger sections of tissue—sometimes including blood vessels—are transferred to restore both form and function.

Post-Procedure Care & What to Expect:

  • Recovery timelines differ depending on the type and extent of reconstruction. It’s common to experience mild to moderate swelling, bruising, and temporary discomfort during the initial days. Stitches are generally removed within the first week.
  • You will be provided with comprehensive aftercare instructions, and our team will support you through each stage of recovery. Results continue to refine over the following weeks to months, and in some cases, additional treatments—such as scar management or laser therapy—may be recommended to further optimize your outcome.
